Full‑time position located in Ardmore, PA.
What you’ll do- Maintain up‑to‑date knowledge and implementation of RHD programs.
- Be familiar with all program‑related regulations from funders, community support services, government licensing agencies, and other related resources.
- Build and enhance relationships with RHD program staff and outside providers to coordinate care.
- Maintain a tracking system to identify the need for comprehensive treatment plans.
- Interface with in‑house and external partner providers through monthly/quarterly off‑site visits.
- Provide direct supervision to Care Coordinators and Resource Coordinators.
- Facilitate routine, comprehensive treatment team collaborative planning meetings.
- Provide statistics and outcomes/measures on quality of care.
- Update and respond to weekly and monthly reporting requirements.
- Review client charts to ensure access to all available resources through systematic analysis.
- Develop comprehensive treatment plans for clients accessing two or more services across settings.

- High school diploma with three (3) years of relevant work experience (required).
- Bachelor’s Degree, preferably in the Human Service field (preferred).
- Two years of experience working with children and/or adults with Mental Health Diagnosis, lDD, or Drug and Alcohol Treatment.
- Valid driver’s license.
