OverviewSame Team partners with Family Nurse Practitioners who own or are building insurance-based private practices to support patients enrolled in the Collaborative Care Model (CoCM).
This is a clinical partnership, not employment
. You remain fully independent and retain clinical authority. Same Team provides Behavioral Health Care Management (BHCM) services
, care coordination, and support that can increase appropriate new patient volume.
Same Team currently supports two care pathways:
- Wellness Coaching (building healthy habits around sleep, stress, exercise, diet, etc)
- Psychedelic Integration Coaching (following legal ketamine or integrative psychiatry treatment)
It benefits your private practice in the following ways:
- Increase new patient volume from patients referred by our clinic partners
- Increase monthly revenue to practice by $5k per month per 100 active patients enrolled in the collaborative care services that we deliver
- Offer your patients virtual wellness program to make healthy lifestyle shifts over 3-6 months
Same Team works with family practices and licensed ketamine and integrative psychiatry clinics to provide structured, longitudinal support (typically 3–6 months) to patients to make healthy lifestyle shifts.
Care focuses on measurable behavioral health and lifestyle goals such as:
- Stress and emotional regulation
- Sleep and energy
- Nutrition and exercise
- Relationships, routines, and purpose
- GAD-7, PHQ-9, WHO-5
When appropriate:
- Patients are referred to your practice to establish or continue care
- Eligible patients are enrolled in Collaborative Care (CPT 99492–99494)
- You serve as the billing practitioner with ongoing clinical oversight
- Same Team delivers BHCM services at a flat rate fee for service
- Psychiatric consultation is provided by our PMHNP as needed
All care is delivered in alignment with CMS Collaborative Care standards

- Serve as the billing practitioner for Collaborative Care services
- Establish care and determine eligibility for CoCM
- Maintain ongoing clinical oversight and reassessment
- Review and act on psychiatric recommendations or escalation needs
- Submit monthly Collaborative Care billing
- BHCM staffing and workflows
- Scheduling and care coordination
- Measurement-based care tracking (PHQ-9, GAD-7, WHO-5)
- Documentation support aligned with CoCM requirements
- Increased new patient volume without marketing or hiring
- Ability to offer Collaborative Care without building internal infrastructure
- Insurance based revenue tied to ongoing patient support
This model is a strong fit for FNPs who:
- Own and are growing an insurance-credentialed private practice
- Are credentialed with Medicare and major commercial payers
- Are comfortable overseeing behavioral health care in a team-based model
- Are excited by providing more support for patients to make healthy lifestyle shifts
- Are open to caring for patients receiving legal ketamine or integrative psychiatry treatment
- FNP-C or FNP-BC
- Licensed in Arizona, Colorado, or Texas
- Credentialed with major insurers or willingness to credential with Alma and Headway
- Comfortable with telehealth and Collaborative Care models
