Position Summary

This position is located at the Sage Memorial Hospital (SMH) under the Medical Staff Department. The incumbent is a licensed physician who provides patient care in the Inpatient, Outpatient, and Emergency Departments  incumbent provides comprehensive, continuous primary care for patients of all ages. Maintains confidentiality of all privileged information. Complies with the Medical Staff Bylaws and the Rules & Regulations.

Essential Duties & Responsibilities
  • Interviews and examines patients; reviews post-medical history; and requests and/or performs diagnostic tests and examinations deemed necessary to obtain preliminary diagnosis.

    Directs, prescribes, or provides treatment or arranges for specialized care for patient referrals, as required. Makes observation notes and incorporates them into clinical records. Provides emergency care, as required. Provides definitive management in all medical categories for cases that do not require referrals. Provides total patient care including prevention, health maintenance, early diagnosis, treatment, and follow-up services to patients under the incumbent’s care.

    Provides medical care across the entire human lifespan, from newborns to geriatric patients.
  • Patient volume expectations in Outpatient – The practitioner is anticipated to attend to two (2) patients per hour during their standard shift. This results in a total of sixteen (16) patients over an 8-hour shift and twenty (20) patients over a 10-hour shift. These projections reflect an estimated average number of patient encounters and are not intended to serve as maximum or minimum thresholds.

    Providers may not end their shifts early or refuse patients at any time unless explicitly authorized by the Outpatient Clinic Director.
  • Refers patients to appropriate contracted medical or other facilities and provides full clinical information for care and diagnostic procedures that cannot be adequately provided ures timely follow-up care, as required.
  • Assures preparation of appropriate medical records for all patients seen to assure accumulation and organization of all needed pertinent clinical data to provide comprehensive medical care. Prepares clinical reports and correspondence concerning patients under his/her care to physicians, hospitals, and to other individuals and facilities.
  • Educates patients and family members on ways to maintain and improve their health status.
  • Supports the Patient-Centered Medical Home (PCMH) concept.
  • Participates with management in planning, instituting, evaluating, and revising medical program plans; assists in determining conditions, resources, and policies essential to delivery of health services.
  • Participates in teaching activities for the various clinical specialties and other health care providers (Physician Assistants, Licensed Independent Practitioners, etc.) as required.
  • Works closely with other licensed independent providers, allied health providers, and other health care professionals (nurses, social workers, behavioral health consultants, Community Health workers, case managers, etc.) in the management of patients’ medical, psychological, and social problems.
  • Participate and attend medical staff meetings and functions.
  • Accept and carry out medical staff committee assignments.
  • Participate in quality assurance activities of the medical staff.
  • Participate in and attend in-service and educational programs seeking to improve and/or learn job-related skills.
  • Precept medical students and residents, as necessary.
  • Present continuing medical education programs to medical staff and to non-physician staff.
  • Strives to maintain and represent the mission, values, and goals of the organization.
  • Ensures adherence to SMH regulatory standards, policies & procedures, and processes.
  • Maintains confidentiality at all times.
  • Perform other duties as assigned.
Minimum Qualifications
  • M.D. or D.O. degree from an accredited school.

    Residency in Family Medicine.
  • Current, active, full, and unrestricted medical license in designated area of medical subspecialty.
  • Board Certified or Board Eligible preferred through the American Board of Family Medicine.
  • Current and Valid DEA certification.
  • Current BLS, ACLS, and PALS certification.
  • Valid Driver’s License.
  • Must be able to successfully pass the Employee Health Program requirements and background investigation.
