9 clinics throughout Maricopa County (Phoenix and surrounding areas) and are continuing to grow as the demands for quality healthcare continues.
Centers provide care to a diverse patient population in a Patient Centered Medical Home model through multi-disciplinary care teams. Joint Commissioned Accredited and affiliated with local medical schools.
