Job Title: Physical Therapist (School Based)
Duration: 52 weeks
Shift: 8am - 5pm, Monday - Friday
Local Pay Rate: $65/hour
Travel Package: $31 (Taxable) + $1372 Stipend = $2612 per week

Responsibilities- Review student’s medical records, physician referrals, child study team referrals, parent requests, and/or school psychologist evaluation recommendations for further testing.
- Perform evaluation procedures to establish performance guidelines.
- Provide formal written comprehensive test results of student assessment within 30 days from the date of evaluation.
- Plan, prepare, and establish appropriate written Individual Education Plan (IEP) goals and objectives.
- Attend student’s scheduled IEP meetings.
- Administer and implement physical therapy/occupational therapy programs during 12 site visits scheduled throughout the period of performance.
- Re‑assess and revise programs as necessary, based on the changing status of the student.
- Instruct and teach students, families, and teachers as indicated on the IEP.
- Make referrals to other health professionals and integrate physical/occupational therapy programs with other aspects of health care.
- Confer with physicians and other health practitioners to obtain information and make recommendations for revisions in treatment programs.
- Supervise and train education technicians and/or certified teachers in occupational/physical therapy assistance to be carried out when the therapist is not at the school.
- Provide weekly on‑site services during the school year, as warranted by student needs.
- Provide progress reports for individual students coinciding with the school’s grading periods.
- Attend contract provider meetings with the Agency Special Education Office.
- Provide a copy of state certification to Kayenta Boarding School within weeks of the beginning date of service.
- Inform the school of the specific provider, who must have an approved security background investigation clearance.
- Be aware that additional service days or any changes to the contract/obligation will not be allowed without receiving modification approval from the contracting officer.
- At least 1 year of experience working with children with disabilities
