Elk Ridge Community Health is seeking an Advanced Practice Pharmacist to serve as the Pharmacist in our 340B Federally Qualified Health Center outpatient pharmacy located within Grand Canyon National Park. This role offers clinical autonomy and the opportunity to practice at the top of your license while serving a rural and underserved population within a Patient‑Centered Medical Home model.
- Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.
- Deliver evidence‑based, patient‑centered drug therapy recommendations and collaborate with the interdisciplinary care team.
- Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.
- Identify, prevent, and resolve medication‑related problems; support patient safety, quality improvement, and clinical guideline initiatives.
- Serve as the primary clinical pharmacy resource for patients, providers, and staff.
- Apply 340B program knowledge to ensure compliance, affordable medication access and sound pharmacy operations.

To become a part of our dedicated team, you need the following qualifications:
- PharmD or equivalent doctoral degree – Required
- 3–5 years outpatient or retail pharmacy experience – Preferred
- BLS certification with in‑person skills component – Required upon hire
