Rounding in Skilled Nursing Facilities (SNF) 10-15 patients per day Responsible for H&P, admission, progress, transfer, discharge, and acute/follow-up Supportive Environment:
Work with an interdisciplinary team Coordinate care transitions to home, independent living centers, assisted living, and/or memory care Collaborate with hospice, palliative care, and home care agencies Professional Development:
