About the Position

Travel Nurse Source  is seeking a qualified Case Manager RN for a 13‑week travel contract at United Health Care Staffing in Fort Defiance, AZ. The assignment begins April 6, 2026, with a weekly schedule of 8:00 AM – 5:00 PM, Monday through Friday. Pay is $67 per hour.

Responsibilities
  • Coordinate patient care across the Surgery Outpatient Clinic, ensuring smooth transitions and timely follow‑up.
  • Collaborate closely with nursing staff and providers to create and implement case management plans.
  • Manage outpatient case loads with attention to detail, prioritizing patient safety and outcomes.
  • Communicate effectively with patients, families, and the care team to clarify care plans and next steps.
  • Document care coordination activities and maintain accurate patient records in a timely manner.
  • Support clinic workflows to reduce barriers to care and improve patient experience.
Qualifications
  • Active Registered Nurse (RN) credential.
  • Minimum three (3) years of case‑management experience (required).
  • Experience in outpatient or surgery clinic settings preferred.
  • Strong collaboration and communication skills with providers and nursing teams.
  • Ability to work the posted weekday daytime schedule and attend scheduled orientations/compliance deadlines.
Knowledge & Skills
  • Case management and care coordination best practices.
  • Patient‑centered care and outpatient clinic workflows.
  • Clinical documentation and care plan management.
  • Ability to communicate and collaborate with multidisciplinary teams.
  • Familiarity with surgical outpatient processes and patient follow‑up.
