Position: Nurse Practitioner - 1099
Location: Chinle

Life Health, LLC, a federal government contract company, is seeking a special Nurse Practitioner with a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree for a 1099 contract position, providing rural nursing health support in any of three locations throughout Arizona:
Kayenta, Chinle, or Page.

Position Summary

This position is both clinical and administrative and works to support across multiple locations in Arizona.

The position is full time from 8:00 am – 4:30 pm, Monday – Friday with Saturday and Sunday off duty within one of the locations specified above, being positioned in alignment with need and service request for the duration of the two‑week on period.

The position will allow for two weeks of on‑time work followed by two weeks of being off‑duty. On‑time work may be assigned to any clinical location, in which you will be expected to orient and obtain security privileging. As these locations require two weeks on duty, if it is not feasible to travel home at weekends or at the end of the day, accommodation including hotel and rental car can be provided.

Nurse Practitioners will act as independent contractors (1099) with Life Health, LLC and must have a FEIN.

In this role, you will:
  • Provide primary medical evaluations to include, but not limited to, patient assessment and medical record review.
  • Develop treatment and care plans based on the most effective and evidence‑based approach for the patient population.
  • Provide chronic disease management services.
  • Develop and/or follow protocol‑based healthcare plans for geographically separated patients.
  • Work collaboratively with all healthcare continuum providers, professional and support staff as required.
  • Initial review and interpretation of laboratory, pulmonary function and ECG studies.
  • Order and administer medications, including vaccines.
  • Order and interpret diagnostic studies as indicated.
  • Deliver medical services via face‑to‑face and/or telehealth settings as required by the provider and its clients.
  • Carry out the full range of work in the specialized area and are professionally responsible for the soundness and validity of recommendations, reports, and services.
  • Complete orientation and training, including one day orientation, two training days emphasizing pain management and women's health, followed by one week of onboarding at each facility.
  • Perform services at clinical locations as directed, including client sites, mobile medical clinics, mobile per diem clinics, and/or approved telehealth formats.
Medical Documentation Requirements

Complete report package and/or medical documentation shall include the following:

  • A medical report that includes corroborating histories, review of systems, physical findings, results of all diagnostic tests performed, diagnoses completed, and follow‑up treatment plan or consultation as required per the customer and/or state/federal law.
  • Each question or field in the claimant/patient medical history completed appropriately.
  • Documentation utilizing the proprietary software or EMR to generate an electronic examination report or medical report, incorporating all diagnostic test results in a timely manner.
  • Compliance with all HIPAA and HITECH security act requirements for patient confidentiality, including a statement of HIPAA and HITECH acknowledgement provided upon hire.
  • Adherence to the Privacy Act of 1974 regarding the release of patient information, requiring written patient consent.
Qualifications
  • Registered Nurse (RN) with a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
  • Current, unrestricted license to practice as an NP in the state in which they practice.
  • DEA prescribing authority in the state of practice.
  • Degree obtained from an accredited program (ACEN or CCNE).
  • Active, unrestricted state medical license.
  • Minimum 5 years’ experience as a Nurse Practitioner.
  • United States citizenship.
  • Current certification in Basic Life Support (BLS) for healthcare providers and CPR for the professional rescuer or equivalent within thirty days of employment.
  • Completion of National Agency Check with Local Agency and Credit Check (NACLC) with SF86 and security privileging for each location.
  • Valid, unrestricted driver’s license with proof of clean background check.
  • Proficiency with electronic health records, patient portals, and healthcare software.
  • Detail‑oriented, analytical, organized with strong time‑management skills, and proficient in Microsoft Office applications.
Compensation

Compensation is paid via a daily rate of $1,100.00, being contractually tied in pricing to exclude negotiation based on qualification or experience.
