Job Title

This position is located in the Kayenta Service Unit's, Division of Nursing, Outpatient/Pediatric Department in Kayenta, AZ.

The role provides case management and clinical coordination for patients and responsibilities include patient assessment, care coordination, service integration, follow-up, and evaluation to improve continuity of care, patient flow, and overall clinic services.

A REAL  be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

Responsibilities:

• Provides outpatient nursing care and patient assessments for individuals across all age groups, including triage, treatments, medication administration, monitoring, and response to changing patient conditions.
• Coordinates case management and continuity of care by arranging referrals, specialty appointments, follow-up care, transportation resources, and communication with internal and external healthcare facilities.
• Manages chronic disease care and patient tracking systems by maintaining disease registries, monitoring patient follow-up, obtaining outside medical records/results, and supporting quality improvement initiatives.
• Provides patient and family education and support through health counseling, disease prevention education, treatment guidance, and assistance in understanding medical conditions and care plans.
• Collaborates with multidisciplinary healthcare teams including providers, nurses, specialty clinics, community resources, and support services to improve patient outcomes, safety, and healthcare delivery.

• On-call hours or standby duty may be required.

Qualifications:

BASIC REQUIREMENT:

Education:

A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Degree from Foreign Nursing School:
Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

Licensure:
For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

MINIMUM QUALIFICATIONS:

Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks:

• Professional nursing knowledge and skills in ambulatory care, patient assessment, case management, care coordination, chronic disease management, patient education, clinical documentation, and referral coordination.
• Must be able to demonstrate the ability to provide nursing care to diverse patient populations, communicate effectively with patients and healthcare teams, recognize and respond to changing patient conditions, operate clinical equipment, and collaborate with multidisciplinary providers to support continuity of care.
