RISE has an opening for a Service Coordinator to oversee our Home and Community Based Services (HCBS) and Managed Care (MC) programs. The Service Coordinator is a safety‑sensitive position and provides leadership and support to Direct Support Professionals and the clients and families we serve. The ideal candidate has experience working with individuals with intellectual and/or developmental disabilities and the elderly.
Responsibilities- Manage and support HCBS programs, implementing RISE mission, vision, and values and delivering hands‑on impact.
- Supervise Direct Support Professionals (DSP) staff.
- Establish partnerships with RISE customers, including clients, families/guardians and funding agencies.
- Oversee and manage program records.
- Prepare and maintain payroll and billing documentation accurately and within designated time frames.
- Ensure programs meet licensing/survey requirements and contractual compliance.
- High School Diploma or GED and at least two years of supervisory experience.
- Fluent bilingual Spanish speaker in both verbal and written communication.
- Experience working with the elderly and/or physically disabled in a long‑term setting.
- Ability to work flexible hours and self‑directed with minimal supervision.
- Travel within the county where HCBS services are provided or interstate travel may be required.
- Must possess a valid Arizona driver’s license, be at least twenty‑one years of age, and meet driver requirements specified by RISE Insurance Agency.
- Must successfully complete a background check, driving record check, drug test, and obtain a Level One Fingerprint Clearance Card.
