EMPACT-SPC is seeking a dedicated Case Manager to support the SMI adult population from our San Tan Valley location. The role provides direct crisis intervention, transportation and education to families and children, and coordinates referrals to partners as needed.
Requirements include a bachelor's degree in a behavioral health field, experience with severely mentally ill adults, flexible scheduling, fingerprint clearance or eligibility, a valid AZ driver’s license, and bilingual Spanish
