Physician (MD​/DO) for In-home Visits

Overview

Signify Health (part of CVS Health®) is looking for physician (MD/DO) contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. The role is flexible, allows you to work one‑on‑one with patients, and pays competitively.

• Conduct in‑home visits focusing on patients’ overall health and well‑being to identify barriers to care.
• Complete 6–9 visits daily (average), earning $1,000–$4,500 per week ($100 per completed assessment with base rate).
• Increase per‑visit rate up to 135% through our market multiplier.
• Earn additional compensation via a pay‑per‑test model for qualifying visits.
• Participate in short‑term or long‑term contracts with no night or weekend duties.
• Pre‑scheduled visits at patients’ homes.
• Compensation may include airfare, hotel, and rental car (depending on eligibility).
• Malpractice coverage and availability incentives based on weekly availability.
• Be an independent contractor (1099 status) with a flexible schedule.

• State medical license required;
  Medicaid license or willingness to enroll strongly preferred.
• MD/DO physician with a background in primary care or family medicine.
• Valid Medicaid license or willingness to obtain one.
• Ability to travel to patient homes (including short‑ or long‑term assignments).
• Strong communication skills and patient‑centered approach.
