Essential Functions and Performance Responsibilities
  • Provide vision diagnostic services for appropriately referred children three to twenty-two years of age in either a format for pre-school or school-age students to meet the mandate of IDEA of a "diagnostic appropriate to the handicapping condition".
  • Be a member of the preschool or school age multidisciplinary team that determines eligibility for visually impaired services by reviewing findings, interpreting medical information and providing educational implications for the results of functional educational vision assessment for district, educational team and parents. Will provide adaption for testing by team members, as necessary.
  • Provide assistance, in initial counseling of parents regarding the identified visual impairment and its implications on learning
  • Be a member of the three year case study re-evaluation team for identified visually impaired students providing a functional vision assessment for determination of continued eligibility.
  • Complete all necessary paperwork related to each referral in a clear, concise manner, following case procedures.
  • Review, usually by a phone contact, specific cases with district or Gavin nurses/technicians for follow-up regarding special services diagnostic referral per Illinois Department of Public Health guidelines.
  • Be available as a source to Vision staff, educational teams, special education staff for information, equipment and materials available for the visually impaired.
  • Assist in provision of an updated library/file of appropriate information meetings.
  • Act as a liaison to the medical community of ophthalmologists and optometrists, as requested.
  • Maintain contact with group of consulting doctors regarding specific issues and specific students, as needed.
  • Maintain updated medical records (ocular reports) for students being serviced
  • Function as an advocate in the provision of a support system for those serving the identified visually impaired at local and regional levels.
  • Provide pertinent information to supervisor of visually impaired programs as to current best practices in the field.
  • Collaborate with educational teams regarding appropriate assistive technology for students with visual impairment.
  • Provide follow-up as needed to specific tasks presented.
  • Coordinate required paperwork to the Illinois State Board of Education.
  • Coordinate the registration of the annual Federal Census of the Legally Blind for students enrolled in one of the Gavin programs.
  • Maintains daily attendance and punctuality
  • Perform other appropriate responsibilities as needed for the betterment of the program.
  • Perform other duties as assigned
