Center Well Home Health is seeking a Home Health Occupational Therapist Assistant to help patients regain strength, mobility and independence in their own homes. You will work under supervision of the Occupational Therapist and collaborate with a multidisciplinary team to deliver rehabilitative care.
Responsibilities include implementing therapy plans, designing adaptive equipment when possible, teaching home exercise programs, and documenting progress in patient records.
