The Community Health Worker is part of the care team charged with creating and integrating patients into health programs to assist with targeted health outcomes. The Community Health Worker will develop, utilize, and integrate community program and resources as part of the development of an effective program. The Community Health Worker directs and is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc.
and in which the practice focus is based on a Patient Centered Medical Home model of care.
Specific duties and responsibilities include:
The Community Health Worker reports to, is supervised by the Director of Care Management.
The evaluation of work performance will be ongoing and will be completed by the Director of Care Management. Specific duties and responsibilities of this position description plus employee ability to interact constructively with clinical and administrative staff and general working behavior. Formal evaluation will be conducted annually but may be initiated at other times by the Director of Care Management.

Qualifications:
Associate's degree or higher in health or human services preferred
Possession of a valid Arkansas Driver's license is required. Independent travel is required. Position will be up to 80% travel within the MHSI service area and occasional meeting outside the area.
This is a FLSA Non-Exempt position. This is an OSHA low risk position. This position is ADA negotiable. This is a safety sensitive position. ( Arkansas Act 593 of 2017)
