Job Description What you’ll do at DBH- Coordinate assessments
- Develop and implement treatment plans
- Provide interventions with acute more complex clients
- Maintain necessary documentation
- Complete & maintains client records
- Complete & maintains discharge documents
- Coordinate referrals
- Plan & implement continuances of care from hospital to community by functioning as linkage for treatment team with case management system through ongoing regular collaboration
This is an hourly position covered by the Ohio Health Care SEIU/1199 bargaining unit (union), with a pay range of 10 on the Ohio Health Care SEIU/1199 Pay Range Schedule
. Normal working hours are 8:00 am - 4:30 pm. This position is located within our Northwest Ohio Behavioral Healthcare location, located at 930 South Detroit Avenue, Toledo, OH 43614
.
Unless required by any applicable union contract and/or requirements of the Ohio Revised Code, the selected candidate will begin at Step 1 of the pay range schedule listed above, with an opportunity for pay increase after six months of satisfactory performance and then a yearly raise thereafter.
