Private Duty Nurse LVN: Weekend Nights

Position Overview

The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician's orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.

• San Antonio, TX 78251, 78242, 78245, 78207 and surrounding areas

• Delivery and coordination of quality patient care in compliance with physician orders.
• Continuously observe and assess patient condition and care needs and report changes to supervisor and/or physician.
• Document all activities, assessments, nursing actions and care coordination in a timely manner.
• Participate in implementation and updating of the nursing care plan.
• Take appropriate nursing action based on assessment and achieve expected outcomes.
• Recognize changes in patient needs and intervene to prevent or reduce risk.
• Accept responsibility for personal and professional accountability by complying with company policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
• Provide care using infection control measures that protect staff and patients under OSHA standards.
• Educate patient and family about disease process, self‑care techniques, and prevention strategies.
• Maintain knowledge of nursing competencies through educational programs, continued education units, and skill evaluations.

• Graduate of an accredited nursing school.
• Current, unrestricted state license as an LPN/LVN in the state of practice.
• Current CPR certification with hands‑on component.
• Demonstrated proficiency in clinical assessment, documentation and compliance with nursing care policies and procedures.
• State‑specific requirements:
  • South Carolina – one year of pediatrics experience.
  • California – one year of experience working under a current nursing license.
  • Louisiana – one year of experience working as a licensed nurse.
  • Continuing education as required by state.

• Six months of recent experience as a Licensed Nurse in a clinical setting.
• Home‑health experience.

• Ability to speak, read, write and understand English.
• Ability to travel.
• Ability to lift 50 pounds.
• Ability to reposition patients and move equipment without assistance.
• Prolonged walking, standing, bending, kneeling, reaching, twisting.
• Ability to sit and climb stairs.
• Visual and hearing acuity.
• Strong sense of smell and touch.
• Ability to respond physically and mentally to emergency situations in the home or during transport.

• Function in a variety of environments that may involve exposure to allergens.
• Possible exposure to blood, bodily fluids and infectious diseases.
