Position Overview
The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician's orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Location- San Antonio, TX 78251, 78242, 78245, 78207 and surrounding areas
- Delivery and coordination of quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs and report changes to supervisor and/or physician.
- Document all activities, assessments, nursing actions and care coordination in a timely manner.
- Participate in implementation and updating of the nursing care plan.
- Take appropriate nursing action based on assessment and achieve expected outcomes.
- Recognize changes in patient needs and intervene to prevent or reduce risk.
- Accept responsibility for personal and professional accountability by complying with company policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
- Provide care using infection control measures that protect staff and patients under OSHA standards.
- Educate patient and family about disease process, self‑care techniques, and prevention strategies.
- Maintain knowledge of nursing competencies through educational programs, continued education units, and skill evaluations.
- Graduate of an accredited nursing school.
- Current, unrestricted state license as an LPN/LVN in the state of practice.
- Current CPR certification with hands‑on component.
- Demonstrated proficiency in clinical assessment, documentation and compliance with nursing care policies and procedures.
- State‑specific requirements:
- South Carolina – one year of pediatrics experience.
- California – one year of experience working under a current nursing license.
- Louisiana – one year of experience working as a licensed nurse.
- Continuing education as required by state.
- Six months of recent experience as a Licensed Nurse in a clinical setting.
- Home‑health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organizational and communication skills.
- Ability to speak, read, write and understand English.
- Ability to travel.
- Ability to lift 50 pounds.
- Ability to reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Ability to sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Ability to respond physically and mentally to emergency situations in the home or during transport.
- Function in a variety of environments that may involve exposure to allergens.
- Possible exposure to blood, bodily fluids and infectious diseases.
