Schedule:
8am-5pm (flexible); every 6th weekend/holiday rotation

OverviewResponsible for the organization and coordination of information from a variety of sources to develop and implement an individualized, age‑appropriate, collaborative plan of care.
Responsibilities- Coordinate staffing to meet the needs of the residents.
- Appropriately delegate and provide follow‑up to ensure completion of assigned responsibilities.
- Provide timely, continuous assessment and preparation of residents and families for the next level of care.
- Identify and effectively communicate pertinent resident information to the health care team, residents, and families in a clear, concise, and timely manner from one level or location of care to another.
- Document Medicare assessments and interventions.
- Accurately file diagnostic and lab reports in the medical record following procedure.
- Transcribe physician orders and round with the provider, noting orders or data entries to complete.
- Record and chart with accuracy and detail, clinical data received from nursing and/or medical staff.
- Maintain systems of organization, such as the monthly ADL, behavior sheets and BM board.
- Prepare daily Medicare sheets and daily/monthly lab slips, prepare charts for physician rounding.
- Schedule annual medical reassessment, dental and pain assessments.
- Recommend positive changes and improvements within the department and facility to improve quality of care and service.
- Support the living environment for the residents and provide a positive environment for staff.
The individual must be able to work the hours specified and perform each essential job function satisfactorily, including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds.
Required Education, License/Certification, or Work ExperienceLicensed Practical Nurse (LPN) – Board of Nursing. An active license in the state of practice upon hire.
Preferred Education, License/Certification, or Work Experience1‑3 years related experience.
