Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home nursing care, assessments, and coordination for patients with life-limiting illness. You will work with an interdisciplinary team to implement plans of care, educate families, and ensure compassionate, compliant care in patients' homes or related settings.
