Seeking a Nurse Practitioner to provide advanced nursing care within a palliative care setting.

Key Responsibilities Include- Delivering specialized primary care, including prescribing and on-call duties, in collaboration with a supervising physician
- Managing care for patients with complex conditions through comprehensive assessments and interdisciplinary team coordination
- Conducting face-to-face patient visits with proper documentation
- Providing clinical education and acting as a resource for staff and team initiatives
- Supporting performance improvement and contributing to clinical excellence
- Unrestricted state license and certification as a Clinical Nurse Specialist or Nurse Practitioner
- National certification from ANCC or AANP
- Possession of NPI, DEA with prescribing privileges, PECOS, CAQH, Medicare, and Medicaid numbers
- Current CPR certification, valid driver's license, and reliable transportation
- At least one year of NP experience in chronic disease, family, or primary care
Compensation and benefits:
