Job Description

Seeking a Nurse Practitioner to provide advanced nursing care within a palliative care setting.

Key Responsibilities Include
  • Delivering specialized primary care, including prescribing and on-call duties, in collaboration with a supervising physician
  • Managing care for patients with complex conditions through comprehensive assessments and interdisciplinary team coordination
  • Conducting face-to-face patient visits with proper documentation
  • Providing clinical education and acting as a resource for staff and team initiatives
  • Supporting performance improvement and contributing to clinical excellence
Qualifications Required
  • Unrestricted state license and certification as a Clinical Nurse Specialist or Nurse Practitioner
  • National certification from ANCC or AANP
  • Possession of NPI, DEA with prescribing privileges, PECOS, CAQH, Medicare, and Medicaid numbers
  • Current CPR certification, valid driver's license, and reliable transportation
  • At least one year of NP experience in chronic disease, family, or primary care
