The Case Manager coordinates the implementation of Individual Employment Plans (IEPs). This role assesses and monitors the implementation and effectiveness of clinical interventions, conducts individual and group therapeutic sessions with program participants, and coordinates behavior management. The Case Manager discusses areas affecting program participants with related health services, DDD case managers, DVR counselors, Support Coordinators, and families/legal guardians/group home staff. This role fosters input in communications, is aware of HIPAA regarding Personal Health Information (PHI) and recognizes that the highest quality of service is delivered with Hospitality, Respect, Quality, Spirituality, and Responsibility, the core values of the Agency, and maintains confidentiality of departmental records, persons, and activities.

- B.S. in Psychology, VR or related Human services field;
- M.S. preferred with upper experience range.
One (1) to three (3) years prior adult counseling or related work with persons with disabilities. - Valid driver’s license with appropriate levels of insurance; CDL or eligibility.
- Eligible for C.R.C. and willingness to pursue;
Already obtained CRC preferred. - Ability to facilitate and coordinate the work of others.
- Knowledge of non-profit educational/training environment.
- Familiarity with funders such as DVRS and DDD.
- ASL or Deaf Studies preferred.
- Required skills, knowledge, and ability to perform job.
