Job Scope

Clinical Care Managers provide clinical oversight and case management services to Colbert and Williams class members transitioning out of nursing facilities in the Northwest Suburbs and Lake County. Facility locations depend on assignment. They provide assessment, care coordination, and clinical community‑based interventions for mental and/or physical health needs. They create comprehensive wrap‑around service plans and coordinate and monitor services. Treatment services are delivered at flexible times and settings that offer the best potential for positive outcomes, including hospital and nursing home centers, in‑home, and community‑based settings.

Program/Clinical Responsibilities

  • Meet regularly with Class Members at assigned nursing facilities in Chicagoland, primarily in the Northwest Suburbs.
  • Oversee and coordinate all service activities for Colbert and Williams Class Members transitioning out of nursing facilities.
  • Take initiative and be accountable for managing all aspects of transition, including navigating system changes and advocating for Class Members.
  • Complete referral and intake into mental health services at KYC.
  • Provide comprehensive, integrated assessments covering mental/emotional health, physical health, employment, education, social and family relationships, substance use/abuse, spiritual, community integration, and housing needs.
  • Create wrap‑around service plans for class members, using input from Class Members, SMHRF/NF staff, family, and guardians while focusing on needs, desires, and preferences.
  • Communicate clearly with the interdisciplinary team, Class Member, supports, and providers to ensure collaborative achievement of service goals.
  • Provide direct service including skill building for activities of daily living and individual treatment addressing trauma, substance use, and other areas.
  • Provide post‑transition monitoring for 18 months and meet the minimum in‑person visit schedule to ensure all services are being accessed and delivered.

Administrative Responsibilities

  • Identify and complete written reports to IDHS on adverse incidents involving Class Members post‑transition.
  • Complete the transition checklist to ensure all required activities are completed.
  • Build and maintain relationships with nursing facility staff to facilitate access to information and class member involvement in the comprehensive transition program.
  • Participate in team meetings to coordinate and support pre‑transition and post‑transition efforts.
  • Complete all clinical documentation and required reports accurately and in a timely manner.
  • Interpret and communicate center policies and procedures at the service delivery level.
