Registered Nurse – Home Health Clinical Support

Arlington Heights, IL 60004 – Full-time – 3 openings – Estimated pay: $78,000 per year

We are seeking an experienced and compassionate Registered Nurse (Home Health) to join our client’s growing team in Arlington Heights, Illinois. In this role, you will provide high-quality nursing care to patients in their homes, promoting independence, comfort, and recovery. • Provide skilled nursing care in patients’ homes following physician‑directed care plans.
• Perform comprehensive patient assessments and develop individualized care plans.
• Monitor and evaluate patient progress, updating care plans as needed.
• Educate patients and caregivers on disease management, medication, and home safety.
• Document all care accurately and in compliance with healthcare standards and regulations.
• Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.
• Participate in case conferences, training, and performance improvement activities.

• Full‑time, Monday–Friday schedule.
• Flexible daytime hours with minimal weekend shifts per year.
• Includes local travel; travel reimbursement provided.
• Comprehensive 6‑week RN onboarding program.
• This is a Pay Per Visit position.

• Local travel required within the Arlington Heights area.

• Associate degree or higher in Nursing.
• Active Registered Nurse (RN) license in Illinois.
• Minimum of 1 year of professional RN experience (home health preferred).
• Current Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification through the American Heart Association.
• Valid driver’s license, reliable transportation, and auto insurance.

• Experience in home health or community‑based nursing.
• Strong clinical documentation and time management skills.
