Position: RN Adult Day Health
Position Overview

The RN will provide comprehensive nursing care and medication administration to Adult Day Health participants in accordance with individualized care plans, physician orders, and emergent needs. This role requires strong assessment skills, experience with Alzheimer's and dementia populations, and the ability to coordinate interdisciplinary care processes within an upbeat, activity-focused program environment.

Minimum Requirements:

• Active RN license in good standing.
• 1+ year Adult Day Health experience preferred, or Long-Term Care experience.
• Experience working with Alzheimer's and dementia patients required.
• Active BLS
• TB Questionnaire, PPD or chest x-ray if applicable
• Must be at least 18 years of age

Key Responsibilities

• Deliver direct nursing care and administer medications per individualized plans of care and physician orders.
• Document all clinical activity and participant progress in the medical record.
• Coordinate completion and accuracy of all initial and ongoing assessments, including timely completion of MDS documentation.
• Lead the development, review, and revision of each participant's care plan.
• Respond appropriately to any acute changes in participants' health or functioning.
• Communicate organizational mission, goals, and strategic direction clearly to internal staff and external partners.
• Contribute to a positive, high-engagement environment consistent with the facility's activity-heavy model.
