Overview
Summary Of Essential Functions For The Position A Licensed Master Social Worker (MSW) provides social work services to patients on an intermittent basis in their place of residence, in accordance with the physicians order and plan of care. Participates in the development of the plan of care with the patient, representative (if any), and caregiver(s), prepares clinical and progress notes, works with the family, utilizes appropriate community resources, in-service programs and acts as a consultant to other agency personnel when needed.
QualificationsQualifications / Licensure / Certification / Knowledge / Skills / Abilities:
- Licensed in the state of Texas
- One (1) year experience in health care environment required
- Experience with home care or hospice preferred
- CPR certified with American Heart Association or American Red Cross
- Ability to exercise initiative and independent judgment
- Strong communication skills, both oral and in writing, with excellent interpersonal skills
- Must demonstrate good customer relations skills and a commitment to providing quality service
- Acceptance and ability to demonstrate support the core values and goals of Agency
- Valid drivers license, auto liability insurance and reliable transportation
- 100% travel may be required
High School Diploma or GED. A masters degree from a school of social work accredited by the Council on Social Work Education (CWSE) and has one year of experience in a health care .
Working Conditions &Physical Requirements
Works indoors in Agency office and patient homes and travels to/from patient homes. Requires minimal physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and minimal assist in lifting and/or transferring of a 50-pounds. Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
Must be able to cope with mental and emotional stress and demonstrate emotional stability. Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions, and hazardous materials.
