Work location: Texas Health Dallas
Work hours: Primarily scheduled Monday through Friday from 8:30 a.m. to 5:00 p.m. The role also requires flexibility to work occasional evenings and approximately two Saturdays per month to support community outreach activities, mobile health events, and patient services.
This is a grant funded position and subject to the award and continuation of the contract

- H.S. Diploma or equivalent is required. Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred.
- Six (6) months customer service experience either face-to-face or by phone is required.
- BCLS – Basic Cardiac Life Support is required within 60 days of hire.
- DL – Drivers License required upon hire
Strong verbal and written communication skills, ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others, ability to complete required reports.
What You Will Do- Travel across Community Health Improvement (CHI) service area, driving to different sites.
- Develop and effectively manage program timelines to ensure timely completion of program deliverables.
- Establish and manage relationships with community members, organizations and coalitions.
- Helps patients with personal goal setting and care plans.
- Responsible for establishing trusting relationships with patients and families.
- Navigates patients and families through medical and non-medical support services.:
- Works closely with medical care team to ensure coordinated care.
