Primarily scheduled Monday through Friday from 8:30 a.m. to 5:00 p.m. The role also requires flexibility to work occasional evenings and approximately two Saturdays per month to support community outreach activities, mobile health events, and patient services. This is a grant funded position and subject to the award and continuation of the contract Community Health Department Highlights

Here's What You Need H.S. Diploma or equivalent is required. Successful completion of a Community Health Worker formal training program such as from a community college or other education institution is preferred. Six (6) months customer service experience either face-to-face or by phone is required. BCLS – Basic Cardiac Life Support is required within 60 days of hire. DL – Drivers License required upon hire Skills Strong verbal and written communication skills, ability to maintain composure in stressful situations, ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others, ability to complete required reports.

Travel across Community Health Improvement (CHI) service area, driving to different sites. Develop and effectively manage program timelines to ensure timely completion of program deliverables. Establish and manage relationships with community members, organizations and coalitions. Helps patients with personal goal setting and care plans. Responsible for establishing trusting relationships with patients and families. Navigates patients and families through medical and non-medical support services.:
