is seeking a S
terile Compounding Pharmacist
for a top home infusion pharmacy in
San Antonio Texas
. This position is ideal for a pharmacist with sterile compounding or clinical pharmacy experience who is interested in providing patient-centered care in a home infusion setting.
- Assignment:13 week contract
- Schedule:12:00 PM 8:00 PM
Job Responsibilities
- Evaluate patients for home infusion therapy and determine eligibility for home-based care
- Develop, monitor, and adjust individualized medication therapy plans based on clinical assessments, laboratory values, and pharmacokinetic principles
- Prepare, verify, and dispense sterile medications in compliance with USP standards, state and federal regulations, and pharmacy policies
- Educate patients, caregivers, and healthcare providers regarding medication therapy, infusion administration, and proper medication use
- Collaborate with healthcare providers to ensure continuity of care and monitor patient outcomes
- Manage infusion pumps and medication administration equipment
- Maintain quality assurance and medication safety standards
- Provide drug information to patients and healthcare professionals
- Maintain accurate patient and pharmacy documentation
- Support regulatory compliance and medication safety initiatives
Requirements
- Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy
- Candidates must have at least one of the following:
- 1+ year of sterile IV compounding experience
- Immunization, and Medication Therapy Management (MTM) experience
- Completion of an ASHP-accredited Pharmacy Residency
- Completion of an Advanced Pharmacy Practice Experience (APPE)
Required Skills
- Strong clinical knowledge of home infusion pharmacy practices
- Ability to interpret laboratory values and apply pharmacokinetic principles
- Strong patient counseling and communication skills
- Strong attention to detail and commitment to medication safety
- Ability to work effectively with patients, caregivers, and healthcare providers
