Medical City Arlington

Full Time Day Shift, 8:00am - 4:30pm Monday - Friday with some weekends.

New Hires may be eligible for a Sign On Bonus up to $20K.

Job Summary and Qualifications
  • Provide clinical expertise and high‑quality care as a RN Case Manager.
  • Promote patient‑centered care by coordinating the plan of care for patient stays, managing length of stay, ensuring appropriate resource management, and developing a safe and appropriate discharge plan in collaboration with the multidisciplinary team.
  • Facilitate progression and transition of care using established criteria and in conjunction with the multidisciplinary team.
  • Coordinate activities that promote quality outcomes and patient throughput while balancing optimal care and appropriate resource utilisation.
What You Will Do In This Role
  • Perform a comprehensive assessment of psychosocial, medical, and discharge needs of patients/families, and assess available resources.
  • Reassess the patient’s clinical condition as indicated and consider readmission risk, developing strategies to mitigate risk through education and community resources.
  • Coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary health‑care team and the patient’s physician to facilitate a successful transition.
  • Collaborate with Social Services to ensure post‑acute medical needs and level of care are appropriate.
  • Triage timely referral to Social Services when psychosocial determinants of health are identified.
  • Involve patients, families, and significant others to clarify needs and expectations, setting realistic goals.
  • Evaluate progress of care using evidence‑based tools (Inter Qual) throughout the episode of care and elevate issues through the chain of command.
  • Make appropriate referrals to third‑party payers, disease and case‑management programs for recurring patients and patients with chronic disease states.
  • Facilitate patient throughput with a focus on effective care transition, quality and efficiency.
  • Document professional recommendations, discharge plans, care coordination interventions, and case‑management activities for all team members.
  • Align patient needs with available resources to ensure safe discharge and transition.
  • Act as a liaison among physicians, patients/families, hospital staff, and outside agencies.
  • Direct activities to address the needs of underserved patients, including education, assistance programs, and community resources.
  • Demonstrate knowledge of regulatory requirements, HCA ethics and compliance policies, and quality initiatives.
  • Advocate for patients’ rights, needs, and values, ensuring cultural and religious considerations are respected.
What Qualifications You Will Need
  • Registered Nurse with current Texas state license.
  • Associate Degree in Nursing or Nursing diploma required;
    Bachelor’s Degree in Nursing preferred.
  • 3+ years of clinical hospital nursing experience or 2+ years experience in case management.
  • Case‑management certification preferred.
  • Experience with Inter Qual preferred.
