Advanced Practice Nurse Practitioner - Arlington TX

Doctors at Home is seeking Full and Part-time Advanced Practice Nurse Practitioners to join our growing Primary Care House Calls practice in Arlington/Grand Prairie/Mansfield TX. Applicants must live in Arlington and be able to commute to the patients homes.

1 year experience as a NP caring for Geriatric patients, preferably in the home environment is required. Must be able to speak, read, write and comprehend English proficiently.
Only qualified candidates will be considered.

• Performing physical exams and patient observations for Geriatric patients in their home, ILF, ALF, Memory Care, Group Homes and Senior Communities.
• Recording patients' medical histories and symptoms in EMR system.
• Creating patient care plans and contributing to existing ones.
• Ordering, administering, and analyzing diagnostic tests.
• Identifying the need for and ordering medical equipment.
• Diagnosing and treating complex health issues.
• Managing, ordering, assessing effectiveness of medications and other treatments.
• Detecting changes in patients' health and modifying treatment plans, as needed.
• Consulting with healthcare professionals and families.
• Training patients and their families to manage and prevent illness and injury.
• NO ON CALL.
• NO WEEKENDS.
• NO MAJOR HOLIDAYS.
