Overview:

Applesoft Homecare Services LLC is seeking a dedicated and skilled Licensed Vocational Nurse (LVN) to provide high-quality in-home healthcare services to our patients. The ideal candidate will be responsible for delivering skilled nursing care with a focus on wound care, PICC line care, and trach care. This is a part-time position with flexible scheduling, competitive per-visit rates, and mileage reimbursement.

Key Responsibilities:
  • Perform wound care, including assessments, cleaning, and dressing changes, according to prescribed protocols.
  • Provide PICC line care, including dressing changes, line flushing, and monitoring for signs of infection.
  • Administer trach care, including suctioning, cleaning, and tube changes as necessary.
  • Monitor patient condition and report any changes to the supervising RN or physician.
  • Document patient care activities thoroughly and accurately in compliance with Applesoft Homecare Services' standards and regulatory requirements.
  • Educate patients and families on care techniques to promote patient health and recovery.
  • Adhere to infection control and safety standards at all times.
  • Communicate effectively with patients, families, and interdisciplinary healthcare teams to ensure coordinated patient care.
  • Provide compassionate, patient-centered care, maintaining dignity and respect for all clients.
Qualifications:
  • Active LVN license in the state of Texas.
  • Minimum of 1 year of experience in home health, with specific experience in wound care, PICC line care, and trach care preferred.
  • Current CPR/BLS certification.
  • Valid driver’s license and reliable transportation.
  • Strong attention to detail and ability to work independently.
  • Excellent communication and interpersonal skills.
Compensation:
  • Competitive per-visit pay, based on experience and qualifications.
  • Mileage reimbursement.
Schedule:
  • Full-Time, flexible scheduling based on patient needs.
