Overview:
Applesoft Homecare Services LLC is seeking a dedicated and skilled Licensed Vocational Nurse (LVN) to provide high-quality in-home healthcare services to our patients. The ideal candidate will be responsible for delivering skilled nursing care with a focus on wound care, PICC line care, and trach care. This is a part-time position with flexible scheduling, competitive per-visit rates, and mileage reimbursement.
Key Responsibilities:- Perform wound care, including assessments, cleaning, and dressing changes, according to prescribed protocols.
- Provide PICC line care, including dressing changes, line flushing, and monitoring for signs of infection.
- Administer trach care, including suctioning, cleaning, and tube changes as necessary.
- Monitor patient condition and report any changes to the supervising RN or physician.
- Document patient care activities thoroughly and accurately in compliance with Applesoft Homecare Services' standards and regulatory requirements.
- Educate patients and families on care techniques to promote patient health and recovery.
- Adhere to infection control and safety standards at all times.
- Communicate effectively with patients, families, and interdisciplinary healthcare teams to ensure coordinated patient care.
- Provide compassionate, patient-centered care, maintaining dignity and respect for all clients.
- Active LVN license in the state of Texas.
- Minimum of 1 year of experience in home health, with specific experience in wound care, PICC line care, and trach care preferred.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
- Strong attention to detail and ability to work independently.
- Excellent communication and interpersonal skills.
- Competitive per-visit pay, based on experience and qualifications.
