Millwood Hospital (a UHS affiliated hospital) is a 134-bed mental health facility that provides inpatient and outpatient mental health and chemical dependency treatment. The hospital has a multidisciplinary staff and has been offering care to children, adolescents, adults, and senior adults since 1971. Intake Assessor/CounselorThe Intake Assessor is a licensed counselor who works under the supervision of the Director of Assessment/Referral. The primary responsibilities include providing in‑person risk assessment to assist physicians in determining appropriate level of care, serving as the initial contact with referral sources and potential patients, coordinating the referral and intake process, facilitating admission, and maintaining communication with referral sources, families, patients, and unit staff.
The role also involves collaboration with the Business Office regarding patient financial data, insurance verification, prior authorization, and coordination with the UR Department for patient certification.
Days (7a - 7p)
Job Duties- Document and process inquiry calls in accordance with intake policies.
- Assess and stabilize patients prior to leaving the hospital or admission to hospital as arranged.
- Notify and assist doctor with patient interview on call.
- Obtain administrative approval for all admissions.
- Complete admission paperwork in accordance with hospital policy.
- Complete the intake log with all documented information.
- Make follow‑up calls to those failing to show up for appointments.
- Obtain pre‑certification in accordance with insurance requirements.
- Complete outcome studies forms.
- Demonstrate guest relations by example and counsel employees in need of training.
- Notify administration of problems or significant events.
- Identify safety problems and take corrective actions.
- Obtain medication after hours as ordered by the physician with proper documentation and accountability.
- Ensure the Core Measures Data (HBIPS) Abstraction Tool is completed for all admissions.
- Perform other duties as assigned by the department director or supervisor.

- Education:
Registered Nurse or Master’s Degree in a human relations or related field. - Experience:
Two (2) or more years in intake assessment activities within an acute psychiatric setting, including a minimum of two years’ experience in a psychiatric hospital setting. - Licensure:
Licensed to practice in Texas as a mental health professional (RN, LMSW, or LPC). - Additional Requirements:
Working knowledge of DSM‑IV and the Mental Status Exam, and of the addiction and recovery field. May be required to work a flexible schedule, overtime, and on‑call as needed.
