Hospice Care Partners is seeking a compassionate, driven Community Liaison to bridge hospice services with families, patients, and care communities. This field-based role requires traveling throughout the DFW Metroplex, building strong referral partnerships and coordinating with clinical teams to onboard new patients.
Base salary starts at $55,000 with monthly performance-based incentives. Health benefit stipend, PTO, and auto allowance are provided to support a mission-driven team.
