We are seeking an experienced, compassionate Corporate Social Services Traveler to support our skilled nursing facilities throughout the Virginia Region
. This position is ideal for a flexible and knowledgeable Social Services professional who thrives in dynamic environments and is passionate about advocating for residents and supporting facility teams.
In this role, you will provide on‑site support to multiple centers, assist during transitions, cover leaves of absence, support survey readiness, and ensure regulatory compliance across facilities.
Key Responsibilities:Provide Social Services coverage and support to assigned skilled nursing facilities
Complete psychosocial assessments, care plans, MDS sections, and discharge planning
Support residents and families with counseling, resources, and care coordination
Assist facilities with survey preparation and regulatory compliance
Ensure adherence to federal, state, and company policies
Collaborate with interdisciplinary teams to promote resident‑centered care
Provide mentoring and guidance to facility‑based Social Services staff as needed
Bachelor’s Degree in Social Work or related field required (MSW preferred)
Current Virginia licensure (if applicable)
Minimum 2+ years of experience in skilled nursing or long‑term care
Strong knowledge of CMS regulations and MDS process
Ability to travel within the Richmond region
Excellent communication, documentation, and organizational skills
Flexible, adaptable, and team‑oriented mindset
