Med Star Health -  - Responsibilities:
Provides physical therapy treatment as directed by the physical therapist and in accordance with agency standards.;
Assists the patient and caregiver to plan adequately for discharge and helps prepare discharge plans with referrals to community resources.;
Assists in assessing the patient’s environment and recommending adaptive equipment or home modifications for safety and function.;
Communicates with supervisor and rehab department regarding patient care and scheduling; participates in quality and service improvement activities.;
Provides information to patient regarding functional status and progress; documents medical records per guidelines; assists with patient education and training for discharge.
