Full-time assignment.
Responsibilities:
- The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours.
- Provide ongoing nursing care to hospice patients and support to their families.
- Coordinate patient care across the continuum under the provider's prescribed plan of care, national guidelines, and within the scope of nursing practice.
