Supervising Nurse (LPN) - Part-Time

SYNERGY Home Care of Arlington/Alexandria is seeking a compassionate, detail-oriented, and experienced Supervising Nurse (LPN) to support our mission of providing quality, client-centered care s part-time position plays a vital role in ensuring the health, safety, and well-being of our clients by overseeing personal care services and supporting caregivers in the field.

The Supervising Nurse conducts in-home supervisory visits, evaluates care quality, ensures compliance with care plans and regulatory requirements, and serves as a key resource for both clients and caregivers. This role includes participation in weekly care conferences, monthly care management meetings, documentation, and follow-up activities.

Key Responsibilities

As the Supervising Nurse, you will:

  • Schedule and conduct supervisory visits with clients and caregivers in the home setting.

  • Evaluate caregiver compliance with care plans and established standards of care.

  • Assess client condition and identify the need for reassessment or care plan updates.

  • Observe caregiver skills and provide coaching, guidance, and support as needed.

  • Monitor client and caregiver satisfaction and identify opportunities for service improvement.

  • Educate clients and caregivers on quality initiatives and best practices.

  • Complete all supervisory visit documentation in Axis Care within 24 hours of each visit.

  • Adhere to company policies, supervisory visit procedures, and applicable Virginia regulations.

  • Report safety, compliance, or service concerns to the Director of Nursing in a timely manner.

  • Follow up on identified concerns and document all actions taken.

  • Assist with new hire orientation, caregiver training, and field support when needed.

  • Participate in weekly virtual Care Conference meetings and monthly Care Management meetings.

  • Accurately submit weekly payroll and mileage documentation.

  • Serve on the Quality Improvement Committee.

  • Maintain compliance with all Virginia personal care licensure requirements and HIPAA standards.

Qualifications Required
  • Current Licensed Practical Nurse (LPN) license in the Commonwealth of Virginia.

  • Valid driver's license and reliable personal transportation.

  • Ability to travel throughout the service areas.

  • Strong interpersonal, customer service, and organizational skills.

  • Excellent verbal and written communication abilities.

  • Experience working in a regulated healthcare environment with documentation requirements.

  • Proficiency with smartphones, computers, electronic medical records, and documentation systems.

  • Basic proficiency in Microsoft Office, including Word and Excel.

  • Ability to maintain client confidentiality and comply with HIPAA regulations.

  • Strong time management skills and ability to manage multiple priorities independently.

Preferred
  • Experience in home care, personal care, senior care, or community-based healthcare settings.

  • Experience supervising caregivers or direct care staff.

  • Familiarity with electronic care management systems such as Axis Care.

Work Schedule

This is a part-time position that includes:

  • Client supervisory visits conducted in the field

  • Participation in two weekly Care Conference meetings

  • One monthly Care Management meeting

  • Documentation and follow-up activities as required
