Multisystem Youth and Family Case Manager (Human Services Clinician II

The Child & Family Services Division (CFSD) of the Department of Human Services (DHS) is seeking a Multisystem Youth and Family Case Manager to be responsible for supporting children and families who are at-risk of out of home placement and multi system involved, or in need of multi system collaboration. The primary goal is to maintain youth safely in the community at home or with kin.

This employee will be part of a multi-disciplinary treatment team responsible for acting as court liaison to include consultation with judges, Guardian Ad Litem (GALs) and court services staff; facilitating cross divisional and multi-agency staffing's on youth who are multi-system (court, behavioral health, child welfare) involved or at risk of becoming multi-system involved; responding to court notices and requests to the Department, and providing short-term case management.

Short-term case management services may include visits to the home, detention center, residential placements, or other community settings.
