LMSW Case Manager, Social Work

Kaiser Permanente is seeking an LMSW Case Manager to support in-patient social work services. In this role, you'll manage complex patient cases, coordinate care plans, and connect patients and families with community and clinical resources. You'll collaborate closely with interdisciplinary medical teams to support safe discharges, address psychosocial needs, and advocate for patient rights and preferences. This position requires strong assessment, documentation, and crisis intervention skills, along with a deep commitment to patient-centered care in a fast-paced hospital environment.

Responsibilities

• Conduct comprehensive psychosocial assessments for in-patient populations
• Develop and implement individualized care and discharge plans
• Coordinate with physicians, nurses, and allied health professionals
• Provide crisis intervention and short-term counseling to patients and families
• Connect patients with community, financial, and psychosocial resources
• Advocate for patient rights, preferences, and safe transitions of care
• Maintain accurate, timely EHR and case documentation
• Participate in interdisciplinary rounds and care conferences
• Educate patients and families about treatment plans and resources
• Support quality improvement initiatives within the social work department
