Overview

SSM Health Physical Therapy - Day Institute is seeking a Case Manager – PRN. Experience in Physical Medicine and Rehabilitation is strongly preferred. The role involves coordinating and managing comprehensive rehabilitation plans for patients recovering from illness, injury or surgery. It requires working closely with medical professionals, patients, their families to ensure that each receives appropriate care and support throughout their recovery journey.

The case manager will serve as a liaison between patients, healthcare providers, insurance companies, and community resources, facilitating a smooth and effective rehabilitation process that promotes patient independence and quality of life.

• Monitors and assesses patient plan of care from pre‑admission to post‑discharge under general direction.
• Assists patient/family in goal setting to maximize independence in home and community.
• Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.

Professional healthcare degree in either:

• Nursing or Social Work
• Licensed PT, OT, SLP

One year medical rehab case management experience (preferred).
