Colorado State Veterans Community Living Center at Rifle

The Colorado State Veterans Community Living Center (VCLC) at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. The center provides 89 beds of skilled nursing care, including an 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouses, and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing personal, social, physical, and medical needs.

Primary Job Duties:

Daily Nursing Care:

  • Performs basic nursing care: bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, and social interaction to meet residents' needs, including FMP Programs.
  • Assists nursing staff with resident safety, mobility, assistive devices, equipment, and gait belts. Performs CPR as needed.

Safety:

  • Ensures resident and staff safety through proper lifting techniques, environment monitoring, and use of safety devices.
  • Performs basic assessments such as weights, vital signs, bowel and bladder incontinence, and records data.
  • Maintains infection control practices and equipment checks.

Communication:

  • Coordinates with team members to ensure proper resident care and work completion.
  • Informs RNs and LPNs of resident condition changes.
  • Maintains a therapeutic environment through effective interpersonal skills and environment monitoring.
  • Participates in restorative and staff meetings.

Transport:

  • Transports residents for appointments using state vehicles when needed.
  • Attends training and staff meetings; ensures effective shift communication.

Minimum Qualifications:

  • High school diploma or GED, and at least one year of professional CNA experience or similar duties.

Education and Work Experience Substitutions:

  • Valid CNA certification from Colorado or another Nurse Licensure Compact state, or a valid practical nursing license (LPN) from Colorado or a participating state, can substitute for the education and experience requirements.

Conditions of Employment:

  • Background check (Colorado Bureau of Investigations and FBI).
  • TB test clearance.
  • At least 18 years old.
  • Valid Colorado Driver's License.
  • Current CNA license, CPR certification, and ability to perform CPR per guidelines.
  • Must be available to report for shift work, including holidays and weekends, and on-call work.
  • Former employees with disciplinary issues or resignations in lieu of termination must disclose details; prior disciplinary actions may disqualify applicants.
