Referral Coordinator

OnPoint Medical Group is seeking a Referral Coordinator to support coordinated, patient-centered care across our primary and specialty clinics. This role manages incoming referral requests, verifies insurance and authorization requirements, schedules appointments, and ensures accurate documentation in the EHR. The Referral Coordinator communicates with patients, providers, and external offices to track referral status and close care gaps. The ideal candidate is detail-oriented, customer-focused, comfortable with medical office systems, and thrives in a collaborative, team-based healthcare environment.

• Process and track incoming and outgoing medical referrals
• Verify insurance eligibility and obtain required prior authorizations
• Schedule patient appointments with internal and external providers
• Document all referral activities accurately in the EHR/EMR system
• Communicate referral status with patients, providers, and partner offices
• Follow up on pending referrals to ensure timely completion of care
• Maintain HIPAA and confidentiality standards at all times
• Support clinic staff with related administrative and coordination tasks
