Signify Health, part of CVS Health, seeks a Full-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for health plan members, traveling within the Denver MSA and Arapahoe County. Visits last about 45 minutes with one-on-one discussions, medical history review, physical exam, and optional tests; you won't prescribe or change treatments.
Requires NP/PA license and board certification; multi-state licenses preferred.
