In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Pueblo

Job Overview

Signify Health, part of CVS Health®, is recruiting Part‑time Clinicians (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. Clinicians will visit patients in their homes, build trusted relationships, and deliver comprehensive, personalized health assessments.

• Each visit lasts approximately 45 minutes, including medication & medical history review, physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry).
• Clinicians use an iPad and an intuitive clinical workflow.
• Clinicians are not required to prescribe medication, order lab tests, or alter current treatment regimens.

• Visit millions of people nationwide to connect them to the right care for their unique needs.
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).

• Active, unrestricted license(s) in coverage area(s).

• Able to cover primary locations:
  Fremont, Saguache, Custer, Alamosa, Costilla, Huerfano, Pueblo, Crowley, Otero, Las Animas.

Anticipated weekly hours: 29.5.

Pay range: $41.85 to $90.13 per hour (base hourly rate or base annual full‑time salary). Base salary depends on experience, education, geography, and other factors.
