Job Description
Title:
Registered Nurse - Home Health Care
Schedule:
8-hour shifts
Assignment Details:
- Contract Length: 14 weeks

Hours:
40 per week
Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years of experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
Roles & Responsibilities :
- Assess home health and/or hospice patients to identify their needs and incorporate this data into the Plan of Care.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Provide education to patients and families regarding care, medications, home safety, etc.
- Supervise and provide clinical direction to support staff to ensure quality service delivery.
- Monitor assigned cases for compliance with requirements of third-party payers.
- Communicate effectively and serve as a liaison among patients, families, and healthcare team members.
