Optimal Hospice is looking for a Licensed Practical Nurse (LPN) to join their team. As a Licensed Practical Nurse (LPN) at Optimal Hospice, you will play a vital role in delivering high-quality care to our patients. Responsibilities- Provide direct patient care under the supervision of a registered nurse.
- Conduct assessments, monitor patient conditions, and communicate findings to the healthcare team.
- Assist with personal care activities if needed.
- Offer emotional support and companionship to patients and their families.
- Collaborate with other members of the interdisciplinary team to develop and implement individualized care plans.
- Maintain accurate and up-to-date documentation of patient care activities.
- Records clinical/progress notes in client/patient's clinical record.

While performing the duties of this job, the employee is regularly required to talk and hear. The position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift or move items weighing 50 pounds.
TravelThis position requires daily travel. Mileage is reimbursed.
