LPN Job Details:

LPN R esponsibilities:

  • Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
  • Assures residents have up to date care plans per state regulations
  • Monitors and maintains resident charts and clinical progress notes.
  • Communicates to physicians, staff and family changes in resident’s needs or conditions.
  • Facilitates coordination of resident services with other service providers.
  • Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
  • Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
  • Communicates in resident’s log any information that staff need to be aware of.
  • Provides alternating supervisory visits in coordination with the RN .
  • Works as a team player with all members of the residents’ care team.
  • Supervises medication practices and medical equipment being used in the building.
  • Assures that staff clearly understand and follow all resident care procedures.
  • Shares resident and family concerns with pertinent staff if appropriate .
  • Works with manager to assure emergency procedures are in place

LPN Qualifications :

  • Maintains a current state license as a n LPN ( Licensed Practical Nurse ) per state regulations
  • A minimum of one year's experience in home health, assisted living or long-term care is preferred
  • Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision
  • Exhibits a caring heart with a passion to care for the elderly
