Position

Summary

The part‑time Advanced Practice Nurse provides holistic, evidence‑based primary care to patients in Monmouth County, NJ. This role supports two key programs based at our Asbury Park health center: the LGBTQ Center for Health and Wellness, which offers inclusive, patient‑centered primary and specialty care, and the Ryan White Part C Program, which delivers comprehensive HIV primary care.

Principal Responsibilities
  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co‑workers
  • Provides direct patient‑centered services to primary care patients at the identified location(s) within established productivity guidelines
  • Maintains complete patient records using the Electronic Health Record and complies with all requirements for billing, coding, and reporting
  • Maintains a clinic panel of people living with HIV and other patients appropriate to ongoing infectious disease oversight
  • Prescribes or administers medication, therapy and other specialized medical care to treat or prevent illness, disease or injury
  • Prescribes and manages gender affirming hormone therapy for transgender patients
  • Provides health and wellness advice; orders age‑appropriate screenings, immunizes patients against preventable diseases
  • Ensures compliance with the New Jersey Department of Health Ambulatory Care Regulations for Licensure as well as requirements of other licensing and accrediting bodies, health maintenance organizations and insurance carriers
  • Collaborates in team‑based care in the Patient Centered Medical Home model
  • Participates in Peer Review and other Quality Improvement activities
  • Collaborates in the provision of in‑service education for staff
  • Adheres to CHC infection control and safety policies, as well as guidelines of regulatory agencies
  • Participates in community outreach programs regarding CHC services and programs as needed
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
Job Competencies &

Minimum Qualifications
  • Licensure as an Advanced Practice Nurse in NJ
  • Master’s degree in Nursing from an NLN accredited program
  • Certified as a Family Nurse Practitioner with prescriptive authority; maintain an active CDS and DEA license
  • 2+ years of primary or ambulatory care nursing experience required. Business or management degree or experience preferred with demonstrated leadership skills
  • Car and valid driver’s license
  • Experience in treatment of people living with HIV or experience working with LGBTQ+ individuals
  • AAHIVS certification preferred
Working Conditions / Physical Demand

Working indoors. Exposure to blood, body fluids and communicable diseases. Sitting, fine motor hand movement, standing and walking. Client/customer contact.

Pay Transparency

In compliance with New Jersey’s Pay Transparency Act, the total compensation range for this position is USD $ to USD $. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate’s work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add‑on incentives, special clinical compensation, retirement plans, and all other benefits.
