Care Coordinator, Embedded
Providing on‑site support within one or more partnered long‑term care facilities in the Ashburn, Virginia area. This role is integral to Ennoble Care’s clinical care team and focuses on continuity, relationship‑building, and patient‑centered support.
Key Responsibilities- Coordinate care for long‑term care residents using Ennoble Care’s EMR.
- Maintain regular, proactive on‑site engagement with residents to support care plans, follow‑ups, and care transitions.
- Collaborate closely with Ennoble Care providers within the long‑term care setting.
- Serve as a key liaison between providers, facility staff, residents, and families.
- Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.
- Accurately document patient interactions and care coordination activities.
- Follow established workflows, documentation standards, and escalation protocols.
- Contribute positively to team collaboration and strong facility partnerships.
This is a fully on‑site role; you will physically work within partnered long‑term care facilities, building relationships with residents, families, providers, and staff. The work is focused on family‑practice‑style care, with strong emphasis on remote patient monitoring and proactive care coordination.
Requirements- Legally authorized to work in the United States.
- Active, unrestricted clinical certification or licensure such as:
- Patient Care Technician (PCT)
- Certified Nursing Assistant (CNA)
- Medical Assistant (MA) / Certified Medical Assistant (CMA)
- Licensed Practical Nurse (LPN)
- Registered Nurse (RN)
- Minimum 1 year patient‑facing experience in a long‑term care, skilled nursing, or similar clinical setting.
- Comfortable communicating via Microsoft Teams, email, phone, and in person.
- Ability to cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff.
- Well‑developed organizational and time‑management skills, optimism, and initiative‑driven mindset.
- Strong working knowledge of healthcare terminology and care coordination principles.
- Proficiency with Microsoft 365 (Outlook, Word, Excel).
- Available to work full‑time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an on‑site role in the Reston, Virginia area with primary focus on Ashburn.
- Ability to pass a background check and drug screen.
- No weekends.
- No on‑call.
- Daytime hours.
- Seven company‑paid holidays annually.
- Valid, unencumbered driver’s license.
- Reliable transportation to fulfill duties.

- Travel between facilities may be required.
